BOLD Performance Therapy understands the importance of protecting your privacy. We are committed to following all requirements regarding client confidentiality, privacy and security as set out by the Health Information Act (HIA) and in accordance with both the College of Physiotherapists of Alberta and Chiropractic College of Alberta.

Collection of private health information is required to provide healthcare within our clinic. It is our duty to collect only information required to perform these services including; name, contact information, healthcare number, medical history, and insurance details.

All information collected is encrypted and stored securely. Your private information is only released to authorized individuals with your written consent, unless required to do so by law as outlined in part 5 of the HIA. A full copy of our Privacy Policy is available by request.

It is our policy to respect your privacy and any information we collect while operating our website.

BOLD staff members adhere to legislative requirements set out by the Health Information Act (Government of Alberta, 2020) (HIA). Personal information and the privacy of clients is protected.

All client and website visitor information will not be disclosed without consent or reasonable and lawful notice, except when required or permitted by law.

Although changes are small and minor, the privacy Policy may change slightly over time, at our discretion. We encourage all site visitors to check this page for any changes to this policy. Your use of this site after any changes have been made establish consent and acceptance to any changes.

 

Cancellation Policy

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists' day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 notice, or miss their appointment, will be charged a cancellation fee as outlined in our cancellation policy.
